Licensed Clinician
Two full-time positions available. One in Mission Gorge and one in Escondido.
Description
Heritage Clinic provides a comprehensive and integrated range of mental health intervention programs for seniors to reduce mental health disabilities and support well-being and independence. Services include assisting clients to access government benefits programs for income and housing opportunities, vocational placement, homeless outreach, case management, dual diagnosis treatment, consumer credit assistance, senior social and educational programs, Legal Aid, community outpatient clinics and psychosocial rehabilitation centers.
Provide supervision to other clinicians.
Responsibilities
Provide field-based individual, family and group counseling, targeted case management and adjunctive services to a broad variety of older adult clients with mental illness. Services include, but are not limited to, clinical outreach in the community, networking with referring professionals, crisis management, psychotherapy, mental health case management, helping clients identify and access resources and housing, and assisting in the establishment of benefits.
Services will be provided in the clients’ homes, in the community and in the office. All services provided will be consumer-centered, culturally competent, and will utilize Bio-psychological Rehabilitation principles. Clinician must demonstrate skill in dealing with older adults, be aware of culturally appropriate behavior and its effects on the responses of his/her clients, and be flexible according to the needs of the client.
Must have been licensed for 2 years in a mental health field (e.g., clinical psychology, clinical social work, or registered nursing) and eligible to provide supervision. Experience in gero-psychology is preferred.
Must have appropriate computer and word processing skills and be able to handle multiple simultaneous tasks. Must demonstrate good interpersonal and communication skills, be able to maintain confidentiality, and present a professional appearance. Must possess a valid California driver’s license and automobile insurance.
If essential job skills are equivalent, preference is given to bilingual individuals, particularly those speaking Spanish, Taglaog, Vietnamese and/or Arabic.
This is a full time position and benefits are available after 90 days of employment.

Mental Health Case Manager
One part-time position avaiable in Escondido
This person provides field-based psychosocial rehabilitation and skills building, targeted case management and adjunctive services to a broad variety of older adult clients, including:

Implement a wide variety of psychological interventions to improve clients’ mental health, including crisis management, providing referrals, problem solving, counseling, mental health case management, and accompanying clients to medical appointments or service agencies to secure needed assistance.

Assists clients in improving daily living, social and self-sufficiency activities.

Plan, coordinate and implement activities and events that will provide social outings and assist clients in improving their Activities of Daily Living (ADL), including but not limited to budgeting, shopping, food preparation, personal hygiene, performing basic household chores, using public transportation, etc., at request of clinicians.

Transport and accompany clients to medical appointments as needed.

Network with referring professionals.

Develop ongoing relationships of trust.

Help clients identify and access resources, including housing.

Assist in the establishment of benefits for which the clients are eligible.

Assists clients during emergency situations by determining needs, performing counseling services, or coordinating placement into hospitals, another level of care or other medical clinics if necessary.

Provide referrals and assistance in accessing community based resources for clients.

Complete applications, for Project Payee or The Organizer, Section 8, Medi-Cal initial and annual renewals, Peer Program referrals and IHSS.

Assist clients to make appointments and go to appropriate clubhouses for SSI application assistance. Ensure that clubhouse submits application to CCHEA.

Transport clients’ personal belongings when moving from facility to facility or to storage if in transition. If belongings are going to storage, sort, pack and properly document.

Complete and submit appropriate forms for billing of services in a timely manner.
Services will be provided in the clients’ homes, in the community and in the office. Other duties and responsibilities:
- Provide clinical input to all Heritage Clinic program operations and staffing issues through participation in all staff meetings and clinical meetings (FSP, FCCS, SBCM).
- Participate in infrastructure development and services provision with a culturally sensitive focus.
- On a rotating basis, be available for service provision during evenings and weekends.
- Bill a minimum of 5.85 hours per day.
- Maintain appropriate San Diego County Mental Health Services documentation.
- Provide clinical outreach services to older adult community residents with mental illnesses.
Qualifications:
Must have a bachelors degree in a mental health field (e.g., psychology, social work,rehabilitation counseling) and at least four years of appropriate experience and training. Up to two years of graduate professional education may be substituted for the experience on a year for year basis. Up to two years of post associate arts clinical experience may be substituted for the required educational experience .Experience in gero-psychology is preferred.
